In collaboration with interdisciplinary clinical team, the Nurse Care Coordinator
will provide assessment, planning and treatment services to adolescents and
adults who are experiencing a first episode of psychosis.
1. Screening, referrals, conducting initial assessments, establishing
therapeutic partnerships, treatment planning, delivery and evaluation
2. Integrating medical and rehabilitation approaches of practice into service
delivery through application of psychosocial rehabilitation principles
and psychiatric nursing skills and knowledge
3. Demonstrating ability to work as a team player within the interdisciplinary
team model of care, through collaborative practice, respectful approaches
to all disciplines/backgrounds represented and openness to input and feedback
• Bachelor of Science in Nursing (BSN) Degree or Practical Nurse Diploma
• Registered Nurse or Registered Practical Nurse in good standing with the
College of Nurses of Ontario
• Understanding of first episode psychosis and best practice approaches
• Good understanding of theory and practice in skills specific to mental health
(i.e. assessment processes, case management, crisis intervention,
psychosocial rehabilitation)
• Strong communication and interpersonal skills; ability to establish and
maintain positive relationships with all staff, clients, hospital and community
• Competency in various computer programs
• Experience working with adolescents / younger adults and with substance
abuse issues is a definite asset
• A valid driver’s license and a $2,000,000 vehicular insurance
• Certification in CAN psychiatric/mental health nursing is an asset
