Nurse Care Coordinator

Permanent Full Time (35 hrs/week)
In collaboration with interdisciplinary clinical team, the Nurse Care
Coordinator will provide assessment, planning and treatment services to
adolescents and adults who are experiencing a first episode of psychosis.
• Bachelor of Science in Nursing (BSN) Degree or Practical Nurse Diploma
• Registered Nurse or Registered Practical Nurse in good standing with
the College of Nurses of Ontario
• Experience in the treatment of psychiatric disorders
• Understanding of first episode psychosis and best practice approaches
• Good understanding of theory and practice in skills specific to mental
health (i.e. assessment processes, case management, crisis intervention,
psychosocial rehabilitation)
• Strong communication and interpersonal skills; ability to establish and
maintain positive relationships with all staff, clients, hospital and
community resources
• Competency in various computer programs
• Experience working with adolescents / younger adults and with
substance abuse issues is a definite asset
• A valid driver’s license and a $2,000,000 vehicular insurance
• Certification in CAN psychiatric/mental health nursing is an asset
